Cthulhu has arrived there and will inevitably ruin everyone's day, but the Doctor happened to be in the neighbourhood and, armed with his trusty sonic screwdriver, the TARDIS and all his friends, will do whatever he can to save the people of Earth. The Doctor wins if he can do so and Cthulhu wins if he can't. They are both in character. HP Lovecraft is a real-life figure in the Whoniverse, and his works are fiction-- the Old Ones still exist, though, and it's stated that he collaborated with the Doctor. Ok, well, i was thinking of the version of Cthulhu from Lovecraft's own books, but i suppose if there are no major differences between the two versions then it doesn't matter much. Cthulhu has arrived on the Whoniverses Earth, but the Doctor has access to all of his gadgets and gizmos and old friends, so he has a good chance here, and has apparently done so before.
